Cronometer is a good one. It's a free web application for tracking nutrition and health data. It lets you log your diet, exercise, biometrics and notes. You can also access it from your apple or android smartphone. There are lots of features. I highly recommend checking it out. Another one you can look into that I've heard good things about (but I've personally never tried) is FitDay.
